We are supposed to collect petitions from different places on different forms. With the old districts, we have only two separate places – Fairfax city and Fairfax county.
For each of these you can download a zip file which has two images, one for the front of the page and one for the back.
Also we have a set of instructions that each person who collects signatures should read, about how to get the forms notarized etc.
